2017 BWA Summer Intensive

All students will need to fill out the Ballet West Academy Summer Intensive Student Packet and Physician's Statement by clicking the links below.

The physician's statement will need to be printed out and taken to your doctor to be completed. Once complete, it must be faxed or mailed to Ballet West Academy by June 1st, 2017. Our fax number and address are on the Physician's Statement form. 

Students staying in our housing will need to fill out the Summer Intensive Resident Packet after they have completed their Student Packet.  

Please fill in as much information as you have available. If you have changes that need to be made before the program begins, please re-submit the form with the dancers first and last name along with the information needing to be updated.
